Be a part of tradition as Northside Hospital-Forsyth presents the 25th Annual Chamber Membership Golf Tournament at Polo Golf and Country Club.

Entry Fee: $175/player; $600/four-player team

Registration: 9:00am
 Start Time: 10:00am

Entry fee includes Green Fees, Cart Rental, Lunch, Beverages, Participant Gift & Awards Reception.
By signing up as a team of four, you can save $100 dollars!
Enter your own team or the Committee will pair singles to make foursomes. 
There is a USGA maximum handicap of 25 allowed. Without a USGA handicap the maximum is 18.
